Host Homes

Host Home Support (HHS) provides a home-like setting that offers support, supervision, training, and assistance for adults aged 18 and older in a certified residential setting or other certified private homes
HHS services include daily supports to maintain individual health and safety, and assistance with activities. Services give individuals with disabilities an alternative to institutional or community living settings. Services rendered under the HHS service code afford adults an opportunity to enhance their ability to live as independently as possible and fully participate in a community setting of their choosing, and to avoid isolation in their homes and communities. 
These services are offered by contract with HHS/DSPD only.  Private pay clients are not an option at this time.

Professional Parent

A professional parent is a state-certified caregiver that provides in-home support to an individual Under the age of 18, diagnosed with a developmental disability or mental health condition.
PPS services include daily supports to maintain individual health and safety, and assistance with activities of daily life. Services rendered under the PPS service code afford children or youth with exceptional care needs an alternative to institutional settings in order to enhance their ability to live as independently as possible and fully participate in a community setting of their choosing, and to avoid isolation in their homes and communities.

These services are offered by contract with HHS/DSPD only.  Private pay clients are not an option at this time.

Eligibility / Criteria

Each potential client will be evaluated based on Behavioral, medical, transportation, mobility, and personal care support needs for appropriate placement in the specific homes that are available at the time of clients referral.

Costs of services

All services are paid for through a contract with HHS and PPS and the rate of pay is determined by the state agency not by BrookeRidge Farms.
